Transfer checklists

Table of contents
  1. For all patients
  2. For mental health patients
  3. For “critical”/unwell patients

When transfering a patient (either interhospital or RFDS) consider the following:

For all patients

  • Patient comfortable and secured on ambulance stretcher
  • Patient belongings
  • Patient notes pack/envelope
  • Handover of care has occurred (or if escorted, planning on transport roles)

For mental health patients

  • Is the patient a voluntary or involuntary transfer?
  • Form 1A and 4A present if involuntary
  • If involuntary, escort present and ready to depart

For “critical”/unwell patients

  • Escort ready to go (has mobile phone, belongings, taxi voucher)
  • Batteries
    • Ventilator
    • Pump(s)
    • Transport monitor (if not using SJA)
  • Arterial line zeroed (if present)
  • Lines neat and untangled
  • Catheter bag and NG bag empty
