Transfer checklists

When transfering a patient (either interhospital or RFDS) consider the following:
For all patients
- Patient comfortable and secured on ambulance stretcher
- Patient belongings
- Patient notes pack/envelope
- Handover of care has occurred (or if escorted, planning on transport roles)
For mental health patients
- Is the patient a voluntary or involuntary transfer?
- Form 1A and 4A present if involuntary
- If involuntary, escort present and ready to depart
For “critical”/unwell patients
- Escort ready to go (has mobile phone, belongings, taxi voucher)
- Batteries
- Ventilator
- Pump(s)
- Transport monitor (if not using SJA)
- Arterial line zeroed (if present)
- Lines neat and untangled
- Catheter bag and NG bag empty
